Patient Forms

New Patients - Garner Internal Medicine now uses PHREESIA for our patient check-in system.  If you provide your email address to us at the time you schedule your initial visit, you will receive an emailed pre-registration invitation 1-3 days prior to your visit.  From this email, you can securely enter your demographic and insurance information before your appointment.  This will help expedite your check in process when you arrive at the office.

After Hours Care

In the case of an emergency, you should always dial 911.

If you are calling with an urgent medical concern that cannot wait until the next business day, or if you need an emergency medication called in (not including controlled substances), have your pharmacy telephone number ready, call our office number at 919-773-1223 and follow prompts to be connected to our answering service or contact them directly at 757-606-3338.

In addition, if you need after hours or weekend treatment, please utilize one of the many local urgent care offices. Urgent care centers are designed to treat many of the problems seen in primary care offices. They offer treatment on a walk-in basis without a scheduled appointment. In addition, most are designed to care for minor fractures, lacerations requiring stitches, and other minor injuries. Utilizing urgent care centers for these types of complaints can help you avoid costly, time consuming emergency room visits. Below are some of the urgent care centers we work with:

Appointments:

Please contact our office at 919-773-1223 for all urgent appointment needs. Appointment requests for non-urgent or routine needs can also be made by calling our main phone number or through our secure portal once you have set up your secure account. We encourage patients who need follow up appointments with a GIM provider to schedule these when checking out at your current appointment to ensure availability of preferred dates and times. If you cannot keep an appointment, please notify us as soon as possible by contacting us at the number above.

Hours of operation:

Monday - Thursday:  8am-12:15pm & 1:30-5pm
Friday: 8am-12:15pm

Registration:

Each time you visit our office, you will be asked to register with the receptionist. Please notify the receptionist of any change of name or address, telephone number, employment or insurance information. Please provide us with your most current insurance identification card. New patients will be asked to fill out a Patient Information sheet to help us better understand your present and past medical problems.

Calls:

The office staff has been trained to answer many of your questions. If they are unable to answer your question, they will obtain the answer from the physician and relay it to you as quickly as possible. However, for the doctor to interrupt a patient exam to answer every phone call would be unfair to the patient under treatment and cause long delays in the schedule.

Prescriptions:

Prescriptions and refills are issued during office hours only.

Medical Records:

Your medical record is confidential. It will not be given to anyone without your written consent. You must sign a release of information form before we can release your medical records. For additional questions, you may call the medical records clerk at (919)-773-1223

Transition From Pediatric Care:

Garner Internal Medicine is committed to helping patients make the transition from pediatric to adult care. We kindly request that you ask your pediatric provider for your medical records, including your immunization history. To make your transition as smooth as possible, we prefer that you bring in a physical copy of your records at your first visit. If that is not possible please fax your records to (919)-773-1955.
